Schools & Day Programs

Rapha Residential and Community Living provides individualized support to students attending public and private school systems, as well as adult day programs. Rapha Residential and Community Living works collaboratively with the school/program integrative and clinical team, associates corroborate an Individual Education Plan (IEP), an Individual Support Plan (ISP) for a student.

Hospitals & Healthcare Facilities
​
Rapha Residential and Community Living supports patients with complex needs throughout their stay in hospitals and healthcare facilities. Rapha Residential and Community Living works collaboratively with healthcare practitioners and other personnel’s Rapha Residential and Community Living Associates provide support for an individual that requires health and mental health needs but also any individuals that is presenting challenging behaviors.

Community & Residential Facilities
​
Rapha Residential and Community Living are obtainable to both funded and privately operated community agencies in crisis situations and for complex supports needs. Our services are customized to the needs of the program as well as the goals and needs of the individual. We also supply staffs in a situation where staffs are needed.

Family Services; Youth and Adults
​
Rapha Residential and Community Living are delivered in the natural environment of the individual, whether it be a community program or a family home. Goals are established during the intake process and attuned throughout our involvement as needed.

Crisis Management
​
Rapha Residential and Community Living responds to all community and family crisis situations with immediacy. We are available 24/7/365. Rapha Residential and Community Living is prepared to support our community members with accessible service coordination and an extensive team of Associates skilled to safely and effectively support during crisis situations.

Skill Development
​
Rapha Residential and Community Living use an individualized approach to our service delivery which makes goals meaningful and specific to the person we are supporting while also acknowledging the capacity and circumstance of the individual. Support goals may include: life skills, social skills, functional communication, community safety and other skills.

BT Consulting/Person Directed Planning
​
Our team of behavioral therapist are available to assess behaviors and develop individualized programming which will be implemented by our Associates. Our services can be on a consultative and/or on-going basis. Rapha Residential and Community Living offers Person Directed Planning to adults 18+ with developmental disabilities, cognitive, physical, social or communicative delays. Associates trained in Person Directed Planning will support the individual in developing a plan that identifies their hopes and interests and provide supports while implementing their goals.

Training
​
Rapha Residential and Community Living offers a range of customized training opportunities for parents and community professionals.
​
​
Youth Justice
​
Rapha Residential and Community Living support individuals between the ages of 12 and 17 who have gotten into trouble with the law. Our support can be provided during weekends, evenings, free time, and unstructured family time. Our Services can include:

Preparing for and returning home following custody

Living at home under conditions

Attending school or work
-
Family tension and conflict
-
Developing positive peer relations and coping with peer pressure

Managing independent or semi-independent living
-
Assisting youth in receiving specific treatment e.g. drug and alcohol treatment

Associate assistance and coverage for youth justice custody and detention facilities
Additionally,
Rapha Residential and Community Living offers community integration supports which allows Associates to work with the youth in their community after creating meaningful and measurable goals set by the probation officer, the youth and their family and other involved community services. Each goal includes a plan for achievement and regular review dates are provided.
